School & Sports Physical Exams

Physicals for school, daycare, camp or sports leagues, with the form completed at the visit. North Houston, walk-ins welcome, Spanish spoken.

Before starting school, summer camp or soccer season, many programs ask for a recent physical. At Clínica Hispana Cruz 2 we do it without an appointment, go over the form with you and explain every finding to parents and students in Spanish.

What the exam covers

  • Height, weight and body mass index
  • Blood pressure and pulse
  • Vision screening
  • Listening to the heart and lungs
  • Spine, joint and posture check
  • Questions about asthma, allergies, fainting and family health

Forms we complete

We fill out school enrollment, daycare, summer camp and sports league forms, plus the pre-participation evaluation used for school sports in Texas. Bring the original form; if the school only sent you a link, print it before you come.

Tests that are sometimes required

Some schools or programs ask for a TB test or up-to-date vaccines. We review your child's shot record and tell you what's missing; ask about the vaccines available at the clinic. If your child has asthma or allergies, it's a good time to go over their plan and medicines before the season.

Why it's more than paperwork

A sports physical helps catch signs that deserve attention early, such as a heart murmur, high blood pressure or chest pain during exercise. If something like that comes up, we explain which test comes next before clearing the activity, and an electrocardiogram can be done at the same clinic.

To keep the visit quick

Fill out the history section of the form at home and bring your child's glasses if they wear them, along with the vaccine record. We're open every day from 9 AM to 9 PM at 13331 Kuykendahl Rd, Suite 128, Houston, with no appointment and no insurance required.

Frequently Asked Questions

For UIL sports it's required before first competing in middle school and again before the first and third years of high school. Other leagues ask for one every season; check what your school or team requires.

It depends on the form. Some schools accept one exam for both, but sports usually have their own form. If you bring both, they're completed in a single visit.

In most cases, yes. We check that their asthma is well controlled and that they have a rescue inhaler, and we note on the form what the school needs to know.
