Which tests we run
We start with TSH, the signal the brain sends to make the thyroid speed up or slow down. If it's off, we add free T4 and, when needed, T3. When the case calls for it, antibodies are added to help tell whether the cause is autoimmune, as in Hashimoto's thyroiditis or Graves' disease. The sample is drawn right here, along with any other blood tests your checkup needs.
Treatment and follow-up
Hypothyroidism is usually treated with a daily thyroid hormone pill, and the dose is fine-tuned with follow-up labs until TSH is back in range. Hyperthyroidism needs a different approach; if a result or a nodule needs a closer look, we guide the referral to a specialist. If you're pregnant or trying to conceive, let us know: the thyroid is watched more closely during that time.
Details that change the result
If you take biotin or hair-and-nail supplements, mention it before the blood draw, since they can skew the measurement. And if you're already on treatment, take your pill at the same time every day on an empty stomach.

TSH alone doesn't require fasting. If you're also checking glucose or cholesterol on the same trip, then it's best to come without eating. If you already take thyroid hormone, ask whether to take it before or after the blood draw.
In most cases it's controlled rather than cured, and treatment is usually lifelong. With the right dose and follow-up labs, people live a normal life.
It can play a part. An underactive thyroid lowers energy use and promotes fluid retention, but it rarely explains a large weight gain on its own. That's why we also look at blood sugar, habits and other causes.
