This guide explains how to recognize it, when it is urgent, and what to expect at the visit.

What is a urinary tract infection?

It is an infection caused by bacteria that enter through the urethra and multiply in the bladder. When it stays in the bladder it is called cystitis; if it climbs to the kidneys it becomes pyelonephritis, which is more serious. Women get UTIs far more often than men because their urethra is shorter, but it can affect anyone, including children and older adults.

What are the symptoms?

The most common symptoms of a bladder infection are:

• Burning or pain when urinating
• A constant urge to go, even when very little urine comes out
• Cloudy, dark or foul-smelling urine
• Pain or pressure in the lower abdomen
• Blood in the urine (pink or reddish urine)

In older adults the infection sometimes causes no burning and shows up as confusion, weakness or general malaise. In young children there may be a fever with no obvious cause, irritability or loss of appetite.

When should I go to the ER instead of the clinic?

Go straight to an emergency room if you have one or more of these signs, because they may mean the infection has reached the kidneys:

• High fever or chills
• Severe pain in the back or side, around the waist
• Nausea or vomiting that keeps you from drinking fluids
• Urinary symptoms during pregnancy

How is it diagnosed?

With a general urinalysis. You provide a sample at the clinic, it is processed on the spot, and the doctor reviews the result with you in the same visit. The test detects signs of infection such as white blood cells, nitrites and blood.

When infections keep coming back or don't improve with treatment, a urine culture is sent out. The culture identifies which bacteria is causing the infection and which antibiotic it responds to; that result takes a few days.

Tip for the sample: wash your hands, clean the genital area, let the first stream go into the toilet, and collect the mid-stream urine in the cup. That makes the sample more reliable.

What does treatment look like?

If the test confirms the infection, the doctor prescribes an antibiotic in the same visit and explains how to take it. Symptoms usually improve within two or three days, but it is important to finish the full course; stopping early makes the infection more likely to return and the bacteria more likely to become resistant.

In the meantime, drink plenty of water, avoid coffee, alcohol and carbonated drinks, and use a pain reliever if the doctor recommends it. If you are not better after three days, come back for a reassessment.

• Drink water throughout the day and don't hold your urine
• Urinate after sex
• Wipe from front to back
• Avoid douching, scented soaps and irritating products in the intimate area
• Wear cotton underwear and avoid very tight clothing

If you get three or more infections a year, we look for the cause during the visit (stones, incomplete bladder emptying, hormonal changes, poorly controlled diabetes) and consider a prevention plan. If you also have discharge, itching or odor, it may be a vaginal infection rather than a urinary one; our gynecology service evaluates it in the same visit.

Frequently asked questions

Can I treat the infection with water or home remedies alone?
Drinking water helps, but it does not kill the bacteria. Without treatment the infection can climb to the kidneys. The safe approach is to confirm it with a urine test and treat it.

Do I need an appointment?

Does the urine test also detect pregnancy or diabetes?
The urine pregnancy test is a separate test; you can ask for it in the same visit. The urinalysis can show glucose in the urine, which is a signal to check your blood sugar.
